Refer a child
Referring a child to our hospice could not be easier, anyone can do it.
Here at Zoë’s Place, we accept referrals from all family members. All we ask is that consent is given from the child’s primary parent or guardian, and that you fill out a referral form (below).
Those from a medical or clinical profession – e.g. Doctor or nurse practitioner – can also refer a child to us by completing the same form.
